Extract PDF Form Fields
Need a map of a fillable PDF form? This tool lists every field name, type and current value from an AcroForm PDF in a clear table, and you can copy it or download it as CSV — processed entirely in your browser.
Select a PDF that has a fillable form. The file is processed in your browser — nothing is uploaded.
| # | Page | Field Name | Type | Value |
|---|
How to use
- Select your fillable PDF file.
- Click Extract Fields — a list of all form fields (text boxes, checkboxes, radio buttons, dropdowns) will be created.
- View the list in the table, or save it with Download CSV to open in Excel.
Only fillable (AcroForm) PDFs have extractable fields. Scanned or image PDFs have no fields.
Tips
- If the table comes up empty, your PDF is probably a scan — scans have no real form fields to read.
- Field names often use short programmer-style labels; check the Value column to see what each field actually holds.
- Save the CSV first, then match each field name to a spreadsheet column before importing the data anywhere else.
